Please provide the following information at this initial stage:
i. Child’s full name & date of birth
ii. Current school
iii. Proposed entry level e.g. Form 1
iii. Proposed year of entry

When making an application, you are also required to provide a report from his former school along with any certificates/ report cards/etc.
